For a Junior Membership form, please fill out the NAWBA membership form and check the appropriate boxes.  We will also need a printed and signed copy of the hold harmless agreement below.                                                     

NAWBA Junior Handler Hold Harmless and Medical Release Form

I acknowledge that my signature as parent  or guardian provides a medical release agreement to NAWBA and any event hosted by NAWBA and parties included in these events of the named Junior handler without exception.  This authorization shall include all activities, including any and all travel to and from activities.  I further acknowledge that my signature provides a HOLD HARMLESS for NAWBA, NAWBA EVENTS, any NAWBA sanctioned or sponsored event and the hosts/trainers/judges and other officials at such events.  Being the legal parent or guardian of the Junior handler named, I hereby agree to the terms of these releases and do give my full consent for his/her participation in NAWBA programs/activities/trials.  I fully understand that due to the nature of the programs/activities/trials, there is a chance of physical injury.  I agree to assume all risk described and not described herein.  I agree to and hereby do release and discharge NAWBA, any NAWBA event sanctioned or sponsored by NAWBA, hosts / trainers/judges and, their officers, etc. from any all claims, demands, causes of action and suits, or liabilities which might arise from such participation, including acts or omissions constituting negligence.  I further agree to and hereby do release and waive any claim which I may have or acquire individually or as guardian of Participant by reason of the describer program.  I make these waivers and releases to legally bind myself; entrant, my executor, heirs and assigns to the fullest extent now and in the future.
